PRFI’s Health Home Program coordinates health and social services that might be needed for an individual or family. The goal of the program is to reduce the number of trips to the emergency room and reduce the amount of time spent in hospital care. It also facilitates regular care from physicians and other providers.  We offer other auxiliary services, such as locating housing services, additional educational resources, and nutritional support.  We make referrals for essential services and monitor the overall process and service delivery.
